Transportation Form

The Lamar CISD Transportation Form must be completed by all students to provide the transportation method to/from home to campus. The parent Transportation Form closes on August 24. After this date, any new submissions or changes to the transportation method will need to be completed by the campus. Contact your child’s campus to make this change.

Parent/Guardian Instructions 

  1. Log in to Skyward Family Access  
  2. Open Online Forms  
    • Once logged in, navigate to the main menu.  
    • Click on “Online Forms.”  
  3. Select the Transportation Form  
    • Find and click “Fill Out Form” for the Student Transportation Form.  
  4. Review the Introduction Screen  
    • Read the information provided regarding student transportation.  
    • Click “Next” on the right side of the screen to continue.  
  5. Complete the Form  
    • Carefully read each question.  
    • Provide all required information accurately.  
  6. Submit the Form  
    • After completing all sections, review your responses.  
    • Submit the form as instructed on the final screen.
